Meet The Family
Mandi and Graham Willoughby moved to Rhode Island in 2012 from Northern California after Graham accepted a position at Brown University. Mandi brought with her a long-held dream nurtured while in the business of caring for others’ loved ones. Her dream was also inspired by her grandmother who had become frail, but did not want to leave her home. Mandi was determined to serve elders just like her grandmother with more personal, home-like care.
In 2016, the concept became reality when Mandi and Graham purchased and converted a Warwick home at 655 Commonwealth Ave. Commonwealth House was Rhode Island’s first “Care Home” — a unique community of 10 seniors who are assisted in their daily lives as they need it, in a home-like setting that they can love.
Not quite assisted living, not quite home care — a better way of senior living. Mandi’s grandmother would approve.
Mandi, Graham, their sons Ian and Rusty, and their Border Collies Buzz and Sumo also reside in Warwick, Rhode Island.

Our skilled, licensed staff members love the “Care Home” concept as much as the Willoughbys do, and freely engage with our residents as family members. No corporate structure or hierarchy. Just friendly professionals eager to provide the most personalized care possible in a setting with a favorable resident-to-staff ratio of 5:1.

Head of Nursing
Connie has spent a lifetime caring for the elderly and now brings her experience and expertise to Commonwealth House! Connie is the Head of Nursing here a Commonwealth, and she is the key to our signature "Concierge Care" service. Connie can be found checking in with each and every one of our residents regularly, making sure they are healthy and comfortable, and also making everyone laugh and smile. She is known throughout the industry as someone who gets things done, and puts extra effort in advocating for our residents' health and happiness every day!
Claudia Braz
House Manager
Claudia has been an incredible part of our home for seven years, bringing reliability, warmth, and heart to everything she does. Residents and staff alike know they can always count on her. She leads with kindness, patience, and a genuine love for caring for others. Whether she’s supporting a resident through their day or making sure a team member feels heard and appreciated, Claudia consistently puts people first.
Her commitment to excellence goes even further — Claudia is about to earn her Master’s Degree in Healthcare Administration, a remarkable accomplishment that reflects her dedication to growing as a leader and continually improving the care we provide. She is always looking for ways to learn, strengthen our processes, and elevate the level of attention and compassion our residents receive.
Claudia also brings joy and creativity into our home. She loves helping residents celebrate holidays throughout the year, making each occasion special and meaningful. Outside of work, she’s a proud animal lover, a passionate foodie who enjoys exploring great meals, loves traveling, and never passes up a chance to spend time at the beach.
Claudia’s leadership, positivity, and drive to “level up” everything she touches make a lasting impact on our residents, our staff, and our entire community. We are so grateful for her seven years of dedication and can’t wait to see all she continues to accomplish.

We welcomed Bobbie to our staff in summer of 2022 and she has brought joy and enthusiasm to our community ever since she walked through the door! She brings many years of experience as well as boundless energy! Bobbie loves dogs, kids and seniors, and we love her for it!

A classically trained chef and graduate of Johnson & Wales University, Chef Mandi (not to be confused with owner Mandi!) brings both skill and heart to every meal she prepares. 🥗🍲🍛
She has a wonderful talent for elevating the traditional “meat and potatoes” dishes our residents love, transforming familiar favorites into meals that are not only comforting, but beautifully prepared, healthy and full of flavor.🍔
The delicious smells from the kitchen, the sounds of cooking, and the joy around the table are all part of what makes our house feel like home.🏠
Beyond her culinary expertise, Chef Mandi brings warmth and laughter into our home each day. A devoted mom and animal lover,🐶 she drives 45 minutes to be here — a true testament to her dedication and dependability. 🚗

Stacy has been part of the Commonwealth House family since August 2025, and she plays a vital role in overseeing the day-to-day care coordination for our residents. Her commitment to excellence, attention to detail, and genuine compassion help ensure that every resident receives thoughtful, organized, and attentive care.
Stacy is a graduate of the RN Program at New England Institute of Technology, where she distinguished herself as a Dean’s List student and scholarship award recipient. Her dedication to learning and professional growth shows in the exceptional leadership she brings to our team.
Known for being super organized and wonderfully detail-oriented, Stacy keeps Commonwealth House running smoothly—making sure our home is spotless, organized, and that nothing ever falls through the cracks. Her fastidious approach helps create an environment where residents and staff alike can feel supported and confident.
What truly makes Stacy special is the time she spends getting to know our residents. She loves learning what brings them joy and fulfillment, and she thoughtfully incorporates those interests into their daily activities and personalized care.
Outside of work, Stacy enjoys Pilates, reading (she’s a total bookworm!), and spending time with her beloved fur babies, Kotaro and Versace. She is also happily married to her beau Jorrell, who works in the care industry as well—making them a true “care couple” dedicated to helping others both professionally and personally.
